Privacy of Medical Records and Requests for Patient Records

All patient medical and financial information, records and data are kept confidential at Elite Brain and Spine. This information is not disclosed to anyone except to the extent necessary to fulfill patient care. All staff members of Elite Brain and Spine adhere to HIPAA guidelines on patient confidentiality.

Requests for copies of patient records necessitates a Authorization to be signed by the patient specifying the type of records to be disclosed, to whom the patient wants them disclosed and the reason why. These authorizations remain in force for 1 year.
